Low self-esteem questionnaire

This questionnaire is made by me and it’s intended to give you an idea of whether or not it’s likely that you suffer from low self-esteem. This questionnaire is based on my clinical experience; the symptoms most commonly displayed by people with low self-esteem.
 

If you answer yes to four or more questions below, I believe it’s likely that you’re struggling with low self-esteem.
 

1. Do you have self-critical thoughts many times a day, most days? Yes/No

2. Do you often feel inadequate at work or in private relationships? Yes/No

3. Do you often feel a need to prove yourself by taking on more responsibilities at work or at home? Yes/No

4. Do you often take on additional tasks at the expense of your own well-being? Yes/No

5. Do you often adapt what you say and do to meet others’ expectations of you? Yes/No

 

6. Do you often assume others think negatively of you? Yes/No

 

7. Do you have difficulty believing your partner (or best friend, if you don’t have a partner) can truly like you? Yes/No

 

8. Are you afraid of being abandoned by the people you love? Yes/No

 

9. Do you have difficulty believing your friends really want to hang out with you? Yes/No

 

10. Are you afraid of conflict and therefore tend to avoid speaking your mind? Yes/No

 

If you answered yes to four or more of the questions above I believe it is likely that you’re suffering from low self-esteem.
